I think what's happened with the "MyDatabase" is that it was created on a different SQL Server with the slighlty different default collation, and has then been detached and re-attached...

From the "lookup table madness" article, the overiding factor for me was the data integrity issue.
eg. You have CustomerType and ProductType ... these get merged into a single "Types" table!...
