Using SSMS, open the script in a query window with a connection to the instance you're having the problem with. It will only affect the instance you're connected to, nothing else. It does affect the system databases, but again, only in the instance you're connected to.
Run some backups first just to be safe 😉

Just in case someone else is running into this issue. I am running SQL 2005 sp4 and was having DB Mail issues, following this instruction:
" rerun the script "C:\Program Files\Microsoft SQL Server\MSSQL.1\MSSQL\install\sysdbupg.sql""
fixed my issue.
