Book Review: Just Culture

I just finished reading Just Culture, which talks about the challenges of building a system that encourages people to report problems (their own or those of others) that might have an impact on safety. It's a little out of the realm of what I usually read, but I added it to my list just out of curiosity after it was mentioned in the news recently. The book has several interesting examples, one that talks about an airline pilot that should have diverted to another airport due to bad weather, but didn't want to because it would cost the company money to do so (actually, more complex than that, but I think it was a big part of the decision). As I think about the book I'm not sure I liked all the examples, because it focused on the situations where things went wrong. I wonder how often institutions get it right? Still, it's important to remember that whistle blowers are usually liked by no one, yet we need people to speak up when things are going badly wrong - and not just safety, money as well.

I saw that the author has another book on Understanding Human Error which might also be interesting to read, because it's the bad (or unfortunate) decisions that require us to figure out a Just Culture.
