Sorry I would love to help, but cannot understand what you want. You seem to have a report produced and want to know how to produce it.
Is the bitmap a picture sent from someone else (e.g. a request) that you need to produce. If so then :-
- create a tablix and add the base (lowest level) detail to the details row [one with 3 lines].
- using the row group section right click and add a Parent Group, and select the PSC_Category column and add a group header.
- using the row group section right click on the PSC_category and add a Parent Group, and select the Provider column and add a group header.
The tablix now should resemble the bmp (just needs colour).
